Self-declaration form to go outside during state of emergency in Kyrgyzstan

AKIPRESS.COM - Movement of citizens will be restricted from 7.00 am to 8.00 pm with entry of the state of emergency into force. Citizens may go outside to buy products, medicines or visit a hospital.
When going outside, citizens should have ID document and a handwritten self-declaration form that will specify the route from the house to a place a citizen is going.
The form should include the following details:
- First and last name
- Phone number
- Reason why you are leaving home
- Time you are leaving home
- Time you expect to return
- Place of residence
- Address of a place you are going to.
Citizens may visit banks when necessary. They should check working hours of banks prior to leaving home.
